ODPS Ohio Emergency Management Agency Exercise Document Templates

Exercise Document Templates

The documents below are intended to be used to assist in the planning process for exercise development and evaluation. These planning forms are intended for a wide variety of exercises. The evaluation tool can be used for discussion-based exercises.

Completing a report for an exercise or real world event does not have to be a daunting task. Accurately documenting incidents and exercises allows stakeholders and participants to actively engage in an improvement process. Ohio EMA has created two After Action Report/Improvement Plan (AAR/IP) documents:

Both templates utilize the Homeland Security Exercise and Evaluation Program (HSEEP) format to make a simple yet effective way to document outcomes and set up a plan for addressing identified gaps.

If you have any questions about either of these documents please contact Gary Lehman, Exercise Coordinator, at gdlehman@dps.ohio.gov.
